CareTree is an online platform that improves home health caregiver operations and facilitates timely communication with families. CareTree replaces paper records and centralizes information to make it easier to care for aging parents or grandparents.

CareTree provides a ‘HIPAA compliant Facebook’ where users can centrally store and share care notes, medications, calendar, contacts, medical documents, and other health information. CareTree provides a central location where care stakeholders can collaborate, communicate and access personal health records in order to provide the best care in the most efficient manner.
CareTree is an open platform that integrates into Electronic Medical Records to centralize the patient’s information without forcing healthcare workers to disrupt their work flow and reenter data. CareTree also integrates with outside healthcare technologies, such as telemedicine solutions like glucose or blood pressure monitors, to provide a single point of access for this information.
With CareTree, a patient or their family caregiver can know all of the medications they are on from various different doctors. They can see when they are supposed to take them and if there are any interactions. They can keep a list of doctors’ appointments or caregivers schedules to easily arrange for transportation. Care stakeholders such as the family, home health caregiver, and primary care physician can all be automatically alerted if there is a change in condition or if a telemedicine reading exceeds a threshold. Instead of carrying a manila folder, the patient and their care stakeholders have all of their information at their fingertips and are empowered to provide the best care possible.
